  • be aged between 4-18 years
We offer up to 12 weeks of therapy within our outreach services.

To self-refer to either of our CAMHS funded services please complete the form below and tick which service you require.

Once submitted you will be contacted and provided with further information on how the service runs and when an appointment will be available for you.

Once a referral is submitted you will see a confirmation that the referral has been sent from this page. IF you do not see this, you may need to try to complete the form on an alternative device.

Once you have completed and submitted a form you will be sent an email acknowledging receipt of your information. In due course you will receive a telephone call to further assess your needs and the process of being placed with a therapist will be explained to you. Due to the high demand for our services there is a waiting time of approximately 12 weeks.

NB. Professionals or self-referral are accepted however professionals need to provide the contact details of the person/parent/carer of the person they are referring please.
